Job Description:
A company is looking for a Clinical Navigation, Nurse
Practitioner (Per Diem).
Key Responsibilities:
Assess patients' needs, gather clinical history, and provide
guidance to navigate healthcare resources
Deliver high-quality clinical services and monitor care quality to
meet patient satisfaction benchmarks
Evaluate and manage patients with acute, chronic, and mental health
conditions while collaborating on efficiency improvements
Required Qualifications:
Licensed NP/PA provider in good standing with state board
Maintains board certification
At least 3 years of NP/PA experience in a clinical setting
Comfortable providing care to all age groups
Ability to thrive in a dynamic work environment while working
independently
