Job Description:
A company is looking for a Clinical Registered Nurse -
Remote.
Key Responsibilities
Provide telephonic support and coordinate with healthcare teams to
manage total healthcare for participants
Identify at-risk participants and implement interventions according
to disease management guidelines
Facilitate communication among multidisciplinary teams and ensure
continuity of care for participants
Required Qualifications
Current RN license for the state of employment and ability to
obtain licensure in other states
High School diploma and a Diploma, Associate's Degree, or
Bachelor's Degree in Nursing (BSN preferred)
Minimum of 3 years recent clinical experience
Basic knowledge of physiology and chronic diseases relevant to
nursing
Basic computer skills and familiarity with a Windows operating
system
